Inside this directory you will find Cimarron Advantage’s exclusive list of participating providers. The Cimarron Advantage PPO Network has been made available to you through your employer’s contract with HCH Administration. Under your benefits plan, which is administered by HCH Administration, you may have the option to enhance your benefit coverage by using the Cimarron Advantage PPO Network providers listed in this directory. How do I use this directory? This provider directory contains primary care physicians, hospitals, specialty care physicians, and ancillary providers. How do I schedule an appointment with a Cimarron Advantage PPO network provider? Step 1: Select a provider from this provider directory If you see a doctor who is not a Cimarron Advantage PPO Network provider, there is usually a reduction in benefits and you will pay a higher cost for your health care. In some groups where employers have elected an EPO option, no benefits may be available for out of network physicians except in limited circumstances. See your Group Health Plan Benefit booklet for details. Remember that Cimarron Advantage PPO Network providers agree to reduce their charges for medical care. Contact your Benefits Manager at your place of employment or your claims administrator, HCH Administration, for a detailed explanation of your benefit plan. If your current physician is not a Cimarron Advantage PPO Network provider, you may ask your physician to contact Cimarron Advantage ’s Provider Relations Department at (866) 679-0825. What if my physician cancels his/her affiliation with Cimarron Advantage Network? Cimarron Advantage PPO Network physicians are contractually obligated to provide a written notice 90 days before they cancel their affiliation with Cimarron Advantage Network. This should allow sufficient time for you to resolve any existing hospital confinements and other medical treatment plans being provided by the physician. You will also have plenty of time to select a new physician. Who do I call if I want to know if a medical service will be covered? Some services may not be covered under your benefit plan. You or your physician can call your Benefit Manager at your place of employment, or the HCH Administration Customer Service Department for a detailed explanation of your benefits. Call (877) 876-5969. If I have a problem finding a Cimarron Network physician, whom do I contact? Simply contact the Customer Service Department toll free at (877) 876-5969 and a Customer Service representative will assist you. Important! Additions, deletions, and changes in the status of a Cimarron Advantage Provider may have occurred after publication of this directory. Verify with your provider at the time of service that they participate in the Cimarron Advantage PPO network. This provider directory does not guarantee that all services or all providers will be covered under your medical benefit plan. You must consult your health benefit booklet or certificate of insurance for complete details of your coverage, including exclusions and limitations. There may be circumstances in which the network benefits are not available.
