The Cimarron Advantage PPO Network has been made available to you through your employer’s contract with HCH Administration. Under your benefits plan, which is administered by HCH Administration, you may have the option to enhance your benefit coverage by using the Cimarron Advantage PPO Network providers listed in this directory.

What is the Cimarron Advantage Network and how are benefits enhanced?
A Preferred Provider Organization (PPO) is comprised of physicians, hospitals, and ancillary providers who have entered into an agreement with Cimarron Advantage PPO. If you are covered under your employer's PPO benefit plan then you and your covered dependents use these providers so you will pay less for covered expenses. If you are covered under your employer's EPO benefit plan, no benefits may be available for out of network physicians, except in limited circumstances.
